Before you complete it
Gather the MVRs needed for the applicable licensing jurisdictions and period. Match them to the driver and identify the person responsible for the carrier review. A vendor delivery receipt does not document the completed review.
What to record
Identify the carrier and driver, record the actual review date and reviewer, list the records and period considered, and explain the determination or unresolved question. Use the comments space to identify evidence requiring verification. Preserve the original MVRs.
Where it belongs
Keep the completed note with the applicable qualification records. This sheet does not replace the MVR, expand an exception, or make a driver qualification decision for the carrier. Check the linked rule before using it for a driver outside the usual federal scope.
